From September 2026, this referral form replaces any document previously used for the same purpose.

Both named parties shall abide by the terms and conditions of the Service Level Agreement and any additional special terms agreed upon. These will be outlined on the booking confirmation that will be sent to organisations once a referral has been processed, an initial meeting has been held, and a start date has been agreed upon.

*** We require 2 different contacts and numbers for parents/carers, including email addresses, as this is key to our communication with families (feedback and reports home).

**** Schools must email education@mapcharity.org a signed Service Level Agreement 2026/27 - Organisations will only need to do this once. Without a Service Level Agreement, learners will not be able to attend MAP.
